Panchamukha Vishnu Ganapathi Dhyan Mandir is situated at Kankuri Village Road, Shirdi, Maharashtra.

Year 1965 Late R. S. Sharma from Hyderabad saw Shirdi Saibaba within dream and a request for the current temple. Thus in the year 2002 he built Panchamukha Vishnu Ganapathi Dhyan Mandir (Ph.: 02423-256510/257398).
